Printing and filling in new patient forms just prior to your new patient visit with us is helpful in guiding the beginning of your care journey with us.

We come to you—via an in-home or in-office visit or via telehealth. We combine a thoughtful, personal, non-hurried, patient centered approach. It’s not just what we do—it’s how we do it that sets us apart. We have both insurance and self-pay options.

We offer flexible pricing based on time spent and health complexity. After an initial conversation, we’ll provide a transparent quote with no hidden costs.
We accept Medicare, BCBS, and United Healthcare insurances; and, you may pay little to no cost out of pocket if using these insurances. New patient in-home visits start typically at $175 and established patient in-home visits are typically $150 via self-pay option without filing any of the accepted insurances or using a subscription plan. Telehealth visits are $79-125 via self pay option without filing any of the accepted insurances or using a subscription plan.
